Forward head posture is exactly what it sounds like: the head sits in front of the shoulders instead of stacked over them. Because the head weighs 4–5 kilograms, every centimetre it drifts forward multiplies the load on the neck and upper back muscles that have to hold it up. Our job is to change the pattern that created the drift, rather than telling you to “sit up straight” — advice that fails within minutes because the underlying muscle balance hasn’t changed.

What is forward head posture?
A postural pattern where the head migrates forward of the shoulders, dramatically increasing the working load on the neck.
In a well-stacked posture, the ear sits roughly over the shoulder and the deep neck flexors, upper back and shoulder blade muscles quietly share the job of carrying the head. In forward head posture, the deep stabilisers at the front of the neck switch off, the muscles at the base of the skull and top of the shoulders grip to stop the head falling further, and the upper back rounds to accommodate it all.
Hours of screens, phones and driving feed the pattern, but so do old injuries, breathing habits, and even how your eyes and jaw work. The result is a small group of muscles doing overtime around the clock — which is why the tension “always comes back” after a massage.

How forward head posture feels
The pattern announces itself in familiar ways. You may have some of these, not all.
Chronic neck & shoulder tension
A constant band of tightness across the top of the shoulders and base of the neck that massage relieves only temporarily.
Headaches
Headaches that start at the base of the skull and wrap toward the temples or behind the eyes, often building through the day.
Upper back ache & rounding
A burning ache between the shoulder blades, and a visible rounding of the upper back that others may point out in photos.
Jaw tension or clicking
Because the head’s position changes how the jaw hangs, forward head posture often travels with jaw clenching, clicking or TMJ discomfort.
Root cause
Why it happens — the real cause
Forward head posture is not a discipline problem — it’s a division-of-labour problem. The muscles designed to hold your head up have gone quiet, and the emergency backup crew has been on shift for years.
Typically the deep neck flexors (the head’s core) and the lower shoulder blade stabilisers are inhibited, while the muscles at the base of the skull, the upper trapezius, the SCM at the front of the neck and the chest muscles are overworking. A stiff mid-back locks the pattern in from below: if the thoracic spine can’t extend, the head has to drift forward just to keep the eyes level. Screen habits, stress-related breathing into the neck and shoulders, and old whiplash-type injuries all reinforce the loop.
This is why “just stand up straight” doesn’t work — you’re asking switched-off muscles to do a job they’ve been fired from. We re-hire them first.

Do I need a scan or diagnosis before booking?

Can't I just fix this by remembering to sit up straight?

Why does my neck and shoulder tension always come back after a massage?
